payment policy

Payment for services is due at the time the service is rendered. The fee for the initial 60 minute consultation is $250. Subsequent psychotherapy fee is $225/50 minute session. The fee for psychological testing and court related cases is $375/hour.

You may pay by check or cash directly to Dr. Goetze at the end of the session. It is also possible to pay by credit card (plus a processing fee determined by Square).

Time spent in review of documents, correspondence, phone calls, report writing, etc. will be billed at a prorated fee. A receipt will be provided to you for possible reimbursement from you insurance carrier.

24 hour advanced notice is required to cancel or change an appointment. A cancellation or missed appointment fee of $75. will be applied with less than 24 hour advance notice.

What billing or insurance information will I receive?

You will receive a copy of your statement sent to you by e mail. This statement will include all the coded information for your insurance carrier to make a determination of your coverage for your reimbursement.

What if Dr. Goetze is not on our plans list of participating providers?

You should call the 1-800 phone number on the back of your card and speak to someone in member services. Many insurance plans will off our of network benefits reimbursed at a somewhat lower rate than for in-network professionals.

What services are usually covered?

Treatment of emotional/behavioral/mental illness are typically covered. Psychological testing will often need to be pre-certified by your insurance carrier. Relationship or marital therapy is often not considered a covered benefit. Please check with your carrier to determine which services are eligible for your reimbursement.
